高海拔疾病預防性藥物如何使用

避免單日營地(睡眠)海拔增加太多是預防高海拔疾病的發作主要的方式. 在登高之前做高度適應也有幫助. 但目前對於高度適應的時機. 高度適應應該在多高的海拔. 待多長的時間, 可維持多久的效果. 目前並沒有明確的答案.  再來根據過去是否罹患過高海拔疾病, 第一天會上升到多高的海拔. 每天海拔增加的高度. 對此次行程做危險分級.  危險分級參考 預防高海拔疾病 high altitude illness (包括 AMS acute mountainsickness, 高海拔腦水腫 HAPE high altitude cerebral edema, 高海拔肺水腫 HAPE high altitude pulmonary edema) 主要的藥物有三種. acetazolamide (以台灣登山界比較熟知的 丹木斯代表 ). 鈣離子阻斷劑 nifedipine. 類固醇 地塞米松 deXamethasone.  治療高海拔疾病藥物劑量 丹木斯可預防 AMS. HACE, 台灣可取得的丹木斯主要是 250mg 的. 預防性使用, 先找出行程之中海拔會超過 2500 公尺的時間點. 往回推 24 小時, 開始服藥. 如果過去不曾吃過, 建議在平地先試吃, 因為丹木斯會有一些副作用, 包括舌頭的異常味覺, 手指腳趾的麻木感覺. 在平地先使用是要了解藥物在自己身體會出現哪些副作用. 如果無法忍受, 可能不適合在山上服用, 如果有過敏, 可就近找醫療院所看診. 丹木斯每次吃 125 mg. 常見劑量是半顆. 每 8 到 12 小時服用一次. 如果行程的危險分級屬於低度危險. 不太需要使用. 如果行程屬於中度或高度危險. 則考慮藥物預防. 在穩定的海拔, 例如 3500 公尺上下. 如果行程中到達穩定海拔, 預計未來海拔都差不多, 每天營地(睡眠海拔)高度落差不會超過 500 公尺. 在穩定海拔連續服用四天之後如果沒有高海拔症狀, 可考慮停藥. (NEJM 2013 guideline 裡面是寫 2-3 天. WMS 2014 guideline 寫四天), 或者到達最高海拔, 行程要開始下降即可停用.  類固醇 deXamethasone , 在海拔到達 2500 公尺可以開始服用, 連續服用不要超過十天, 類固醇停用...

( Tintinalli's Emergency Medicine ) External ear squeeze 外耳道擠壓症 外耳道如果因耳垢或耳塞造成阻塞, 無法平衡耳膜及外耳道壓力, 導致耳膜向外突出, 稱為外耳道擠壓症.  症狀包括疼痛及耳膜出血.  If the external canal is occluded by cerumen or an ear plug, the inability  to equalize pressure between the external canal and the tympanic membrane  causes the bending of the tympanic membrane outward, producing  an injury called “external ear squeeze” that produces pain and tympanic membrane hemorrhage.

( Tintinalli's Emergency Medicine ) Face, tooth, or dry-suit squeeze   顏面 牙齒 乾式防寒衣擠壓傷 含有空氣的其他結構在下潛的時候會造成擠壓症 面鏡擠壓症: 臉部瘀血, 結膜充血, 結膜出血, 視力改變, 罕見情況還可能造成眼睛後方出血. 眼後方出血可能變成眼科急症 牙齒擠壓症: 因蛀牙或填充物,膿瘍引起. 會造成牙痛 乾式防寒衣擠壓症: 會造成表皮條紋狀瘀血,疼痛.  Other air-containing structures can be compressed during descent, producing  “squeeze” symptoms. A face squeeze occurs when air is not  added to the facemask during descent, causing the face and eyes to be  forced into the collapsing mask. This can produce facial bruising,  conjunctival injection or hemorrhage, changes in vision, and, rarely,  retrobulbar hemorrhage. The latter could be a true ophthalmologic  emergency. A tooth squeeze occurs when air spaces inside a tooth—due  to decay, a filling, or an abscess—become compressed during descent. A  dry-suit squeeze occurs when suit folds are compressed into the underlying  skin, producing local trauma manifested by painful red streaks.
